ABSTRACT

Introducción: El cateterismo urinario es un procedimiento frecuente y en ocasiones es utilizado por fuera de las indicaciones aceptadas para el mismo. Esto aumenta el riesgo de complicaciones vinculadas a su uso, por lo que pueden ser prevenibles. El objetivo del estudio es conocer las características del uso de cateterismo urinario en pacientes ingresados en salas de cuidados moderados de un hospital universitario del tercer nivel de atención, determinar la frecuencia, duración e indicaciones más frecuentes, así como evaluar la presencia de complicaciones asociadas al mismo Metodología: Estudio de corte transversal, realizado en salas de cuidados moderados de un hospital terciario y universitario de Montevideo, Uruguay, el 21 de diciembre de 2022. Se incluyeron pacientes hospitalizados que presentaban o presentaron catéter vesical en la presente internación y se completó la recolección de variables mediante la revisión de la historia clínica. Resultados: De 155 pacientes ingresados en salas de cuidados moderados, a 26 (16,7%) les fue colocado un catéter urinario. La mediana de edad fue 61 años, 80% eran de sexo masculino. La mediana de internación fue de 22 días. En todos los pacientes se utilizó sonda vesical y el 54% fue colocado en el Departamento de Emergencia. En el 46% de los pacientes no se encontró indicación escrita de colocación en la historia clínica. En 50% de los casos no está especificado el motivo de indicación de sonda vesical, mientras que las indicaciones identificadas más frecuentes fueron el control de diuresis (27%) y la desobstrucción de vía urinaria baja (23%). La duración de cateterismo fue de una mediana de 13,5 días, mientras que el 27% de los pacientes la usaron más de 30 días. 35% de los pacientes presentaron complicaciones vinculadas a la sonda vesical, en su mayoría no infecciosas (27%) y 15% presentaron infección urinaria. Estos pacientes tuvieron una duración de cateterismo mayor a los que no presentaron complicaciones (23 vs 10 días, p=0,411). Conclusiones: El catéter vesical fue utilizado en un porcentaje no despreciable de pacientes ingresados en salas de cuidados moderados, de forma prolongada y frecuentemente sin indicación precisa, lo cual expone a un riesgo aumentado de complicaciones vinculadas.


Introduction: Urinary catheterization is a frequent procedure and is sometimes used outside of its accepted indications. This increases the risk of complications related to its use, so they may be preventable. The objective of this study is to know the characteristics of the use of urinary catheterization in patients admitted to moderate care wards of a tertiary care university hospital, to determine the frequency, duration and most frequent indications, as well as to evaluate the presence of associated complications. Methodology: Cross-sectional study, carried out in moderate care wards of a tertiary care and university hospital in Montevideo, Uruguay, on December 21, 2022. Hospitalized patients who present or presented a bladder catheter during the present hospitalization were included, and the collection of variables was completed by reviewing the medical history. Results: Of 155 patients admitted to moderate care wards, 26 (16.7%) had a urinary catheter placed. The median age was 61 years, 80% were male. The median hospitalization was 22 days. In all patients a bladder catheter was used and 54% were placed in the Emergency Department. In 46% of the patients, no written indication for placement was found in the clinical history. In 50% of cases, the reason for indicating the bladder catheter is not specified, while the most frequent indications identified were diuresis control (27%) and lower urinary tract obstruction (23%). The duration of catheterization was a median of 13.5 days, while 27% of the patients used it for more than 30 days. 35% of the patients presented complications related to the bladder catheter, mostly non-infectious (27%) and 15% presented urinary tract infection. These patients had a longer duration of catheterization than those without complications (23 vs 10 days, p=0,411). Conclusions: The bladder catheter was used in a non-negligible percentage of patients admitted to moderate care wards, for a long time and often without a precise indication, which exposes them to an increased risk of related complications.


Introdução: O cateterismo urinário é um procedimento frequente e às vezes é usado fora de suas indicações aceitas. Isso aumenta o risco de complicações relacionadas ao seu uso, portanto, podem ser evitáveis. O objetivo deste estudo é conhecer as características do uso do cateterismo urinário em pacientes internados em enfermarias de cuidados moderados de um hospital universitário terciário, determinar a frequência, duração e indicações mais frequentes, bem como avaliar a presença de complicações associadas ao mesmo. Metodologia: Estudo transversal, realizado em quartos de cuidados moderados de um hospital terciário e universitário em Montevidéu, Uruguai, em 21 de dezembro de 2022. Foram incluídos pacientes que apresentaram ou apresentaram sonda vesical durante a internação atual e a coleta de variáveis ​​foi concluída .revisando o histórico médico. Resultados: Dos 155 pacientes admitidos em enfermarias de cuidados moderados, 26 (16,7%) tiveram um cateter urinário colocado. A idade média foi de 61 anos, 80% eram do sexo masculino. A mediana de internação foi de 22 dias. Em todos os doentes foi utilizada sonda vesical e 54% foram internados no Serviço de Urgência. Em 46% dos pacientes, nenhuma indicação escrita para colocação foi encontrada na história clínica. Em 50% dos casos não é especificado o motivo da indicação da sonda vesical, enquanto as indicações mais frequentes identificadas foram controle da diurese (27%) e desobstrução do trato urinário inferior (23%). A duração do cateterismo foi em média de 13,5 dias, enquanto 27% dos pacientes o utilizaram por mais de 30 dias. 35% dos pacientes apresentaram complicações relacionadas ao cateter vesical, em sua maioria não infecciosas (27%) e 15% apresentaram infecção urinária. Esses pacientes tiveram uma duração mais longa de cateterismo do que aqueles sem complicações (23 vs 10 dias, p=0,411). Conclusões: A sonda vesical foi utilizada em percentual não desprezível de pacientes internados em quartos de cuidados moderados, por tempo prolongado e muitas vezes sem indicação precisa, o que os expõe a um risco aumentado de complicações associadas.

Resumen Objetivo: Determinar los factores asociados, las características clínicas, microbiológicas y perfiles de resistencia en las infecciones sintomáticas de tracto urinario asociado a catéter (ISTU-AC) en los dos Instituciones de alto nivel de complejidad. Materiales y métodos: Se llevó a cabo un estudio de casos y controles. Se incluyeron todos los pacientes con más de 48 horas de hospitalización con inserción de catéter urinario y se recolectaron todas las variables clínicas y microbiológicas de cada paciente. Se incluyeron 446 pacientes, 223 con infección sintomática del tracto urinario asociado a catéter. El análisis multivariado se realizó por medio de regresión logística. Resultados: Se evidenció una mayor proporción de hombres en los controles (60,5%) en comparación con los casos (51,1%), la mediana de la edad fue muy similar para los dos grupos de estudio. Se evidenció por el análisis de regresión logística multivariado que la estancia en UCI (OR 2,176; IC de 95% 1,332 - 3,555), más de 10 días de catéter urinario (OR 2,907; IC de 95% 1,744 - 4,846) y la terapia antibiótica previa (OR 0,060; IC de 95% 0,037 - 0,103) fueron los principales factores asociados con la ocurrencia de ISTU-AC. No se encontró asociación entre presentar el evento de interés y la edad, esta variable no está relacionada con la pre sencia de infecciones intrahospitalarias. Es probable que las comorbilidades, presentes con más frecuencia en el adulto mayor, hayan sobrepasado el efecto de la edad en esos estudios. La asociación con género no fue identificada como un factor de riesgo, lo cual podría esperarse, dado que el uso de la sonda vesical hace equiparable el riesgo entre los hombres y las mujeres. Summary Objective: To determine the associated factors, clinical and microbiological characteristics, and resistance profiles in symptomatic urinary tract infections associated with catheters in the two institutions with a high level of complexity. Materials and methods: A case-control study was carried out. All patients with more than 48 hours of hospitalization with urinary catheter insertion were included and all the clinical and microbiological variables of each patient were collected. 446 patients were included, 223 with symptomatic urinary tract infection associated with a catheter. The multivariate analysis was carried out by means of logistic regression. Results: A greater proportion of men was evidenced in the controls (60.5%) compared to the cases (51.1%), the median age was very similar for the two study groups. It was evidenced by the multivariate logistic regression analysis that the stay in the ICU (OR 2.176; 95% CI 1.322 - 3.555), more than 10 days of urinary catheter (OR 2.907; 95% CI 1.744 - 4.846) and antibiotic therapy previous (OR 0.060; 95% CI 0.037 - 0.103) were the main factors associated with the occurrence of CA-UTI. No association was found between presenting the event of interest and age, this variable is not related to the presence of intrahospital infections. Co morbidities, which are more frequently present in the elderly, are likely to have outweighed the effect of age in these studies. The association with gender was not identified as a risk factor, which could be expected, since the use of the urinary catheter makes the risk comparable between men and women. Conclusions: This study showed that symptomatic urinary tract infections associated with catheters are still a frequent entity in the hospital setting, putting the safety of patients at risk and increasing both morbidity and mortality rates

ABSTRACT

Abstract Objective To evaluate the differences between bladder emptying options (permanent catheterization and intermittent bladder emptying/spontaneous urination) regarding the effects on labor length, need of operative vaginal deliveries, and cesarean section rate. Data Sources The search was conducted in MEDLINE, Scopus, Web of Science, and The Cochrane Central Register of Controlled Trials databases. Selection of Studies The survey returned 964 studies. A total of 719 studies were evaluated by title and abstract, of which 4 were selected for inclusion. Data Collection All references were inserted in the Rayyan QCRI tool (Rayyan Systems Inc., Cambridge, MA, USA). The full text of the selected articles was obtained so we could later decide whether or not to include them in this systematic review. Data Synthesis No differences were found in the number of instrumented deliveries or in cesarean section rate between groups. Conclusions After evaluating the studies performed on the topic, we concluded that there is no clear advantage to either method, although continuous catheterization was associated with a greater occurrence of eutocic births. In the remaining outcomes, there were no differences between catheterization types.


Resumo Objetivo Avaliar as diferenças entre as opções de esvaziamento vesical (cateterismo permanente e esvaziamento vesical intermitente/micção espontânea) em relação aos efeitos na duração do trabalho de parto, necessidade de partos vaginais operatórios e taxa de cesárea. Fontes de Dados A pesquisa foi realizada nas bases de dados MEDLINE, Scopus, Web of Science, e The Cochrane Central Register of Controlled Trials. Seleção de Estudos A pesquisa retornou 964 estudos. Um total de 719 estudos foram avaliados por título e resumo, dos quais 4 foram selecionados para inclusão. Coleta de Dados Todas as referências foram inseridas na ferramenta Rayyan QCRI (Rayyan Systems Inc., Cambridge, MA, EUA). O texto completo dos artigos selecionados foi obtido para posterior decisão de incluí-los nesta revisão sistemática. Síntese dos Dados Não foram encontradas diferenças no número de partos instrumentados ou na taxa de cesariana entre os grupos. Conclusões Após avaliação dos estudos realizados sobre o tema, concluímos que não hávantagem clara de qualquer um dos métodos, embora o cateterismo contínuo tenha sido associado à maior ocorrência de partos eutócicos. Nos demais desfechos, não houve diferenças entre os tipos de cateterismo.

ABSTRACT

INTRODUCCIÓN: El piometra es una afección infrecuente, pero grave, que en general se diagnostica en mujeres posmenopáusicas. En adolescentes es sumamente raro, y si se acompaña de amenorrea primaria hay que tener en mente las anomalías congénitas. CASO CLÍNICO: Adolescente de 13 años, sin antecedentes personales de interés salvo amenorrea primaria, que acude con abdomen agudo y es intervenida por una peritonitis difusa causada por un piometra secundario a disgenesia (estenosis) cervical congénita. Se realizó dilatación cervical y se dejó una sonda vesical intrauterina para prevenir la reestenosis. CONCLUSIONES: Un diagnóstico precoz y un tratamiento conservador con dilatación cervical y colocación temporal de un catéter urinario son esenciales para un manejo seguro y efectivo de la estenosis cervical en adolescentes.


INTRODUCTION: Pyometra is an uncommon but serious condition that is generally diagnosed in postmenopausal women. In adolescents it is extremely rare; if accompanied by primary amenorrhea, consider congenital abnormalities. CASE REPORT: A 13-year-old adolescent, with no relevant personal history except primary amenorrhea, who presented with an acute abdomen and was operated on for diffuse peritonitis caused by pyometra secondary to congenital cervical dysgenesis (stenosis). Cervical dilation was performed and a urinary catheter was temporarily placed inside the uterus to prevent restenosis. CONLUSIONS: An early diagnosis and conservative treatment with cervical dilation and temporary placement of a urinary catheter are essential for the safe and effective management of cervical stenosis in adolescents.

ABSTRACT

Objective:To evaluate the efficacy of continuous infusion of lidocaine via urinary catheter for postoperative analgesia in patients undergoing urological surgery.Methods:Forty male American Society of Anesthesiologists physical status Ⅱ or Ⅲ patients, aged 65-75 yr, with body mass index of 18-25 kg/m 2, scheduled for elective percutaneous nephrolithotomy, were divided into 2 groups ( n=20 each) using a random number table method: continuous infusion of lidocaine through urinary catheter group (group L) and patient controlled intravenous analgesia (PCIA) group (group PCIA). All the patients underwent total intravenous anesthesia, and a matched type of sterile urethral irrigation catheter was inserted after the operation.In group L, 0.5% lidocaine was continuously infused at a rate of 5 ml/h via the urinary catheter, while the equal volume of 0.9% normal saline was continuously infused via the urinary catheter, and PCIA was connected in group PCIA.PCIA solution contained sufentanil 125 μg (diluted to 250 ml in normal saline), and the PCA pump was set up with a 5 ml bolus dose, a 15 min lockout interval and background infusion at a rate of 5 ml/h.When visual analogue score was>4, sufentanil 0.05 μg/kg was injected intravenously as rescue analgesic.The development and severity of catheter-related bladder discomfort (CRBD) were recorded immediately at the end of the operation (T 1), and at 6 h (T 2), 24 h (T 3) and 48 h (T 4) after the operation, respectively.Riker sedation-agitation scale (SAS) score was recorded at T 1, 2, and QoR-9 scale was recorded at T 3, 4.The concentrations of serum cortisol (Cor), norepinephrine (NE), epinephrine (E) and blood glucose (Glu) were measured by enzyme linked immunosorbent assay.First off-bed time, exhaust time, length of hospital stay after surgery, and the requirement for rescue analgesia and adverse reactions (nausea and vomiting, respiratory depression, hypotension, skin itching) within 48 h after the operation were recorded. Results:Compared with group PCIA, the incidence of CRBD and the severity were significantly decreased at T 1-4, SAS score was decreased at T 1, 2, QoR-9 score was increased at T 3, 4, Cor, NE, E and Glu concentrations were decreased at T 1-4, the incidence of postoperative rescue analgesia was decreased, first off-bed time, exhaust time and length of hospital stay after surgery was shortened, and the incidence of postoperative nausea and vomiting, respiratory depression, hypotension, skin itching was decreased in group L ( P<0.05). Conclusion:Continuous infusion of lidocaine through the urinary catheter can provide good postoperative analgesia, reduce postoperative stress response and adverse reactions, and facilitate early postoperative recovery in patients undergoing urological surgery.

ABSTRACT

Aims@#Biofilm formation of bacteria inside the surface of urinary catheters triggers severe urinary tract infections (UTIs). This study aims to determine the biofilm forming capacity of bacteria isolated from urinary catheters of patients diagnosed with UTIs as well as comparison of antibiotic sensitivity patterns between biofilm and non-biofilm forming isolates.@*Methodology and results@#A total of 40 urinary catheters were collected from 96 h catheterized patients. The isolated uropathogenic bacteria were identified and examined for biofilm formation using the microtiter plate method. Later, the isolates were subjected to antimicrobial susceptibility towards 12 antibiotics that commonly used for treating UTIs using the disk diffusion method. All the catheters were found colonized with two to five different bacterial species individually. Out of the 131 isolates from 40 catheters, Pseudomonas aeruginosa (38/131, 29%) was the predominant isolated bacteria followed by Escherichia coli (31/131, 24%), Proteus vulgaris (24/131, 18%), Klebsiella pneumoniae (21/131, 16%) and Staphylococcus aureus (17/131, 13%). Among these, the highest biofilm forming capacity was observed in P. aeruginosa (26%), followed by P. vulgaris (16%) and K. pneumoniae (13%). Regarding antibiotic resistance, biofilm forming bacteria showed resistance to multiple drugs except for carbapenems. Moreover, biofilm formers exhibited higher resistance than non-biofilm formers against antibiotics such as trimethoprim/sulfamethoxazole (100% vs 82%), amoxicillin-clavulanic acid (81% vs 55%), cefixime (85% vs 55%), ceftriaxone (81% vs 45%), cefalexin (93% vs 55%), amikacin (70% vs 45%), ampicillin (89% vs 73%), ciprofloxacin (70% vs. 36%) and ceftriaxone (81 vs 45%), (p-value<0.05). @*Conclusion, significance and impact of study@#Most of the isolated uropathogenic bacteria from catheters were biofilm formers and multiple antibiotic resistant. Appropriate selection of antibiotics, meticulous hygiene practices in hospital settings and limiting the duration of catheterization can reduce biofilm formation and the emergence of antibiotic resistance.

@#Objective    To explore the independent risk factors for postoperative retention of urinary catheters in the ward of lung tumor patients due to urinary retention under the concept of enhanced recovery after surgery (ERAS). Methods    Seventy-five patients with lung tumors who had urinary catheters left in the postoperative ward between June 2019 and August 2019 were selected as a case group, and 75 patients with lung tumors who did not have urinary catheters in the perioperative period as a control group. Independent risk factors for indwelling urinary catheters in the postoperative ward were screened by univariate and multiple-variate logistic stepwise regression analysis. Results    There were 45 males and 30 females in the case group with an average age of 55.33±10.78 years, 28 males and 47 females in the control group with an average age of 57.12±10.06 years. Univariate analysis showed that gender, operative time>2 h, intraoperative fluid volume≥1 200 mL, and fluid volume within 6 h of returning to the ward after surgery>1 200 mL were associated with the occurrence of indwelling urinary catheters in patients with lung tumors in postoperative wards (P<0.05). Multiple-variate logistic regression showed that male (OR=2.311, 95%CI 1.173-4.552, P=0.015), infusion volume within 6 h of returning to the ward after surgery>1 200 mL (OR=2.491, 95%CI 1.149-5.401, P=0.021) and intraoperative infusion volume≥1 200 mL (OR=2.105, 95%CI 1.022-4.340, P=0.044) were independent risk factors for postoperative retention of urinary catheters in patients with lung tumors. Conclusion    The occurrence of indwelling urinary catheter in lung tumor patients under the ERAS concept is the result of a combination of factors, and patients who are male, have infusion volume>1 200 mL within 6 h of returning to the ward after surgery, and have intraoperative infusion volume≥1 200 mL are the high-risk group for postoperative ward indwelling urinary catheter, and health care personnel should strengthen the assessment and observation, provide targeted health education, appropriately control the perioperative fluid volume, and take other measures to reduce the occurrence of indwelling urinary catheters due to urinary retention postoperatively in ward.

Resumen La infección del tracto urinario asociada al catéter urinario permanente (ITU/CUP) es un problema relevante en los centros de salud por su alta frecuencia. Objetivo: Identificar factores de riesgo de ITU en pacientes adultos con CUP. Material y Método: Estudio caso control efectuado entre los años 2010-2016 en el Hospital Militar de Santiago. Se realizó un análisis estadístico univariado y multivariado por medio del modelo de regresión logística binaria, con variables como edad, duración e indicación del dispositivo, algunas comorbilidades y la instalación del dispositivo luego de 15 días hospitalización. Resultado: Se obtuvo un total de 63 casos y 123 controles. Fueron variables predictoras de ITU/CUP la duración del CUP desde el séptimo día en adelante (OR 2,6 IC 1,4-4,9, p = 0,004) y la instalación del CUP con una estadía de hospitalización previa de 15 días y más (OR 7,8 IC 2,920,9 p = 0,000). No se encontró asociación con la edad mayor de 80 años, comorbilidades como diabetes y vejiga neurogénica e indicación de instalación. Conclusiones: Los resultados permiten focalizar las intervenciones, evaluando la necesidad real de indicación de CUP en pacientes con estadía hospitalaria previa de dos semanas y fomentar el retiro de CUP antes del séptimo día de uso.


Abstract Catheter-associated urinary tract infection (CAUTI) is a relevant problem in health centers because of its high frequency. Objective: To identify UTI risk factors in adult patients with urinary catheter. Material and Method: Control case study carried out between the years 2010-2016 at the Military Hospital of Santiago, Chile. Univariate and multivariate statistical analysis were performed using the binary logistic regression model, variables such as age, duration and indication of the device, some comorbidities and the installation of the device after 15 days of hospitalization. Result: A total of 63 cases and 123 controls were obtained. Predictive variables of CAUTI for the duration of the urinary catheter from the 7th day onwards (OR 2.6 IC 1.4-4.9, p = 0.004) and the installation of the urinary catheter with a previous hospital stay of 15 days and more (OR 7.8 CI 2.9-20.9 p = 0.000). No association was found in age over 80 years, comorbidities such as diabetes and neurogenic bladder and indication. Conclusions: The results permitted to focus the interventions, evaluating the real need for indication of CUP in patients with previous hospital statistics of 2 weeks and encouraging the withdrawal of CUP before the 7th day of use.

ABSTRACT

Resumen Introducción: La infección tardía es una posible complicación del implante de un esfínter urinario artificial (método más aceptado para tratar la incontinencia urinaria de esfuerzo secundaria a prostatectomía radical). Si sucede, debe retirarse la prótesis y colocar una sonda vesical hasta su reinserción meses más tarde. El paciente se convierte en portador domiciliario de sonda vesical, que puede adquirir una infección de tracto urinario por un mal cuidado. Objetivo: Desarrollar sesiones educativas para que un paciente adquiera destreza en el manejo de sonda vesical y consecuentemente, prevenir las infecciones asociadas. Desarrollo: Se presenta el caso de un varón de 80 años, portador de sonda vesical por retirada temporal de su esfínter urinario artificial. Al realizar la valoración de enfermería, se identifica el diagnóstico enfermero Conocimientos deficientes en relación con los cuidados del dispositivo. Posteriormente, se seleccionan los resultados esperados y se planifican las intervenciones educativas. Tras llevarlas a cabo identificamos que el paciente incrementó sus conocimientos sobre el manejo de la sonda vesical. Conclusiones: La educación sanitaria es una herramienta de prevención del riesgo de infección asociado al mantenimiento de la sonda vesical en el domicilio, ya que, si el paciente no es capaz de afrontar su autocuidado, el tratamiento pierde efectividad. Es imprescindible identificar las necesidades formativas de los pacientes, abordarlas mediante intervenciones educativas reforzadas con documentación escrita durante el ingreso, para proporcionar la información necesaria y fortalecer su comprensión, por lo que puede mejorar el nivel de autocuidado, así como prevenir complicaciones tras el alta hospitalaria.


Abstract Introduction: Late infection is a possible complication an artificial urinary sphincter´s implant the most accepted method to treat urinary incontinence secondary to radical prostatectomy. If this happens, the prosthesis must be removed, and a urinary catheter was placed. Then, the patient becomes a urinary catheter home carrier prone to acquire urinary tract infections as a result of carelessness. Objective: To develop educational interventions so that these patients acquire dexterity to manage the urinary catheter and thus, prevent the possible associated infections. Development: A case of an 80 years old male carrying a urinary catheter temporarily substituting an artificial urinary sphincter is addressed. The nursing diagnosis assessment is performed. The patient demonstrates Insufficient knowledge related to the management of the device. Educational interventions addressing the situation are planned. After the implementation of the strategy, the patient improves his knowledge of urinary catheter management. Conclusions: Health education is an infection risk prevention tool that can address the management of urinary catheters in-home patients, improving the effectiveness of the related treatments. It is essential to identify the needs of the patients and help address them through educational interventions to enhance self-care and prevent possible complications.


Resumo Introdução: A infeção tardia é uma possível complicação do implante de um esfíncter urinário artificial (método mais aceito para tratar a incontinência urinaria de esforço secundária a prostatectomia radical). Se acontecer, deve retirar-se a prótese e colocar uma sonda vesical até sua reinserção meses mais tarde. O paciente torna-se portador domiciliar de sonda vesical, podendo adquirir uma infeção de tracto urinário por um mal cuidado. Objetivo: Desenvolver sessões educativas para que um paciente adquira destreza no manejo de sonda vesical e consequentemente, prevenir as infecções associadas. Desenvolvimento: Presenta-se o caso de um homem de 80 anos, portador de sonda vesical por retirada temporal de seu esfíncter urinário artificial. Realiza-se a valoração de enfermagem, onde se identifica o diagnóstico de enfermagem Conhecimentos deficientes ao respeito dos cuidados do dispositivo. Posteriormente, selecionam-se os resultados esperados e planificam-se as intervenções educativas. Após levá-las a cabo, consegue-se que o paciente incremente seus conhecimentos sobre o manejo da sonda vesical. Conclusões: A educação sanitária é uma ferramenta de prevenção do risco de infeção associado à manutenção da sonda vesical na residência, já que, se o paciente não for capaz de afrontar seu autocuidado, o tratamento perde efetividade. É imprescindível identificar as necessidades formativas dos pacientes e combatê-las mediante intervenções educativas reforçadas com documentação escrita durante o ingresso, para proporcionar a informação necessária e reforçar sua compreensão, pelo que pode melhorar o nível de autocuidado e prevenir complicações após a alta hospitalar.

RESUMEN Introducción: con el incremento de la esperanza de vida al nacer, la morbimortalidad en la población geriátrica es alta, y con ello la necesidad de atención médica especializada, que incluye los cuidados intensivos. Objetivo: caracterizar a la población geriátrica que ingresó en una Unidad de Cuidados Intensivos (UCI) Municipal. Materiales y método: se realizó un estudio de corte transversal en el Hospital General Docente "Orlando Pantoja Tamayo" en el municipio Contramaestre. El universo estuvo constituido por 658 pacientes de ambos sexos, con edades mayor e igual a 60 años. En el período de enero de 2015 a diciembre de 2018, admitidos en la UCI. Las variables estudiadas fueron edad, sexo, principales diagnósticos al ingreso, ventilación mecánica, procederes invasivos, estado del paciente al egreso y complicaciones. Se utilizó el porcentaje para resumir la información, así como el test Ji cuadrado para identificar asociación estadística entre las variables estudiadas. Resultados: predominó el sexo masculino en 51,8 % y 50,3 % tenían edades entre 60 y 74 años. 73,1 % tuvo estadía inferior a siete días. Predominaron las patologías cardiovasculares. El porcentaje de fallecidos 26,3 %, 58,5 % se le implantó sonda vesical y 63,0 % de pacientes necesitaron de ventilación mecánica invasiva. Prevalecieron los ingresos por enfermedades clínicas. Conclusión: los pacientes geriátricos del sexo masculino ingresaron con más frecuencia en terapia intensiva. Predominó el grupo de edades 60 - 74 años. Las enfermedades clínicas cardiovasculares y respiratorias fueron las más frecuentes, la mayoría de pacientes que fallecieron necesitaron ventilación mecánica invasiva (AU).


ABSTRACT Introduction: with the increase in life expectancy at birth, morbidity and mortality are high among geriatric population, and so are the needs for specialized medical care, including the intensive care. Objective: to characterize the geriatric population admitted in a Municipal Intensive Care Unit. Materials and Method: a cross-sectional study was carried out at the Teaching General Hospital "Orlando Pantoja Tamayo" in the municipality of Contramaestre. The universe was formed by 658 patients of both sexes, aged 60 years and older, during the period from January 2015 to December 2018, admitted in the Intensive Care Unit. The studied variables were age, sex, main diagnoses at admission; mechanical ventilation; invasive procedures; patient status at discharge and complications. The percentage was used to summarize the information, as well as the Chi square test to identify statistical association between the variables studied. Results: male sex predominated in 51.8%, and 50.3 % were aged 60 - 74 years old. 73.1% had a stay of less than seven days; cardiovascular diseases predominated. 26.3% of patients died, 58.5% received a bladder catheter implantation. 63.0% of patients required invasive mechanical ventilation. Admittance due to clinical diseases prevailed. Conclusion: male geriatric patients were more frequently admitted in intensive care; the 60-74 years-old age group prevailed; cardiovascular and respiratory clinical diseases were the most frequent; most of the dead patients needed invasive mechanical ventilation (AU).

ABSTRACT

Introduction: Catheter associated urinary tract infections (CAUTI) is the most common health care associated infection (HAI), accounting for 40% of all HAIs in particularly in patients admitted to intensive care units (ICU) due to their more frequent necessity of urinary catheterization and longer duration of catheter use. Knowledge about Microbiological profile and antimicrobial sensitivity pattern of CAUTI is essential to implement the infection control system in a proper manner.Aims and objectives: To study the rate and microbiological profile of catheter associated urinary tract infections and in patients admitted in Intensive Care Units.Materials and methods: A one-year prospective study was conducted in which urine samples were collected from patients admitted in Medical ICUs on indwelling urinary catheter for >48 hours and processed in Microbiology Department according to standard procedures. CA-UTI rate was calculated and their microbiological profile and various resistance patterns were studied as per CLSI guidelines.Results: Out of 1406 enrolled patients, 64 patients developed CAUTI. Overall CAUTI rate was 4.06/1000 catheter days. Most common isolate in CAUTI cases was Klebsiella spp. (32.3%), followed by Escherichia coli (24.6%). Gram negative isolates showed higher sensitivity to aminoglycosides, imipenam and nitrofurantoin as compared to cephalosporins The Enterococcus spp. were 100% sensitive to linezolid and 71.4% sensitive to both vancomycin and teicoplanin.Conclusion: CAUTI rate in our ICUs was high compared to the benchmarks. Antibiotic sensitivity pattern of the pathogens involved was also low. This study provides the data of predisposing risk factors and its causative microbial flora for CAUTI in our tertiary care.

@#Objective    To investigate the occurrence of indwelling urinary catheter in patients receiving thoracoscopic lobectomy and relevant risk factors. Methods    We retrospectively reviewed the clinical data of the 737 patients who received thoracoscopic lobectomy in our hospital and analyzed the risk factors of indwelling urinary catheter during postoperative hospitalization using univariate analysis and multiple-variate logistic regression analysis between December 2018 and May 2019. There were 253 males and 484 females at median age of 57 (50, 64) years. Results    A percentage of 14.4% (106/737) of the patients adopted postoperative indwelling urinary catheter. Univariate regression analysis showed that gender and postoperative bedridden time were risk factors for indwelling urinary catheter in the patients after thoracoscopic lobectomy (P<0.05). Multiple-variate logistic regression analysis showed that male gender (OR=2.018, 95% CI 1.316-3.096, P<0.001) and postoperative bedridden time >18 hours (OR=2.298, 95%CI 1.502-3.516, P<0.001) were the independent risk factors for indwelling urinary catheter. Conclusion    Male gender and those with longer postoperative bedridden time are high-risk population to indwell urinary catheter. Positive measures should be taken to reduce the chance of indwelling urinary catheter.

ABSTRACT

Clean intermittent catheterization (CIC) is one of the core elements of neurogenic bladder management in the patients with spinal cord injury and is effective and safe to maintain low intra-bladder pressure and achieve urinary continence. Until now, the most notable development in urinary catheter products for CIC is the introduction of hydrophilic coating. Fortunately, in Korea, the national medical insurance has recently covered the cost for urinary catheters in this patient group. The purpose of this review is to summarize the history of CIC and the recent development of urinary catheter products. From our review, we would like to suggest a way of thinking that is the way forward for the future to improve the implementation of CIC with minimal morbidity.

ABSTRACT

Objective To systematically assess the efficacy and safety of non-indwelling urinary catheter during perioperative period of cesarean section.Methods The databases including Medline(PubMed),EMbase,Cochrane Library,CNKI and WanFang Data were retrieved by computer.The randomized controlled trials (RCTs) on the efficacy and safety of non-indwelling urinary catheter during perioperative period of cesarean section were comprehensively collected.Two reviewers independently screened literatures,extracted the data and assessed the quality.The Meta-analysis was conducted by using RevMan 5.3 software.Results A total of 5 RCTs involving 1 090 patients were included.The meta-analysis results showed that compared with the indwelling urinary catheter group,although the non-indwelling urinary catheter group increased the incidence rate of urinary retention[RR=11.67,95%CI(2.22,61.24),P<0.01],but significantly decreased the incidence rate of urinary tract infection(UTI) [RR=0.10,95%CI(0.04,0.26),P< 0.01] and incidence rate of initial urination discomfort [RR=0.17,95%CI (0.04,0.74),P=0.02],significantly shortened the initial time of off-bed activities [SMD=-3.68 ,95%CI(-5.25,-2.12),P<0.01]and hospitalization time [SMD=-1.03,95%CI(-1.67,-0.38),P<0.01].There were no significant differences in the operation time[SMD=-0.13,95%CI(-0.32,0.07),P=0.20] and incidence rate of postpartum hemorrhage (PPH)[RR=1.50,95%CI (0.43,5.26),P=0.53],The incidence rates of bladder injury in the two groups were similar.Conclusion According to the evidence of current clinical researches,selecting non-indwelling urinary catheter in elective cesarean section is effective and safe,prompting that indwelling urinary catheter during the perioperative period of cesarean section is selective rather than routine indwelling.

ABSTRACT

Objective To evaluate the hemostasis effect of balloon urinary catheter after vacuum-assisted breast biopsy (VABB).Methods From May.2016 to May.2017,270 patients undergoing VABB were randomized into study group (135 cases) and control group (135cases).Patients in the study group received VABB postoperative indwelling catheter balloon hemostasis,while patients in the control group received VABB postoperative conventional thoracic pressure bandage to stop bleeding.Postoperative bleeding and hematoma were recorded and compared between the two groups.Results The rates of postoperative bleeding and hematoma in the study group were significantly lower than that in the control group (6.7% vs 16.3%,P<0.05;8.9% vs 24.4%,P<0.05).Among patients with lesions ≤ 1.5 cm,the rates of postoperative bleeding and hematoma were 1.6% and 4.7% in the study group,and 6.5% and 8.1% in the control group.There was no significant difference between the two group (P>0.05).Among patients with lesions >1.5 cm,the rates of postoperative bleeding and hematoma in the study group were significantly lower than those in the control group (11.3% vs 24.7%,P<0.05;12.7% vs 32.9%,P<0.05).Conclusion Hemostasis with balloon urinary catheter is a safe and effective method for postoperative bleeding and hematoma control after VABB.

Objective To reduce the complications of male indwelling urinary catheter and improve patient compliance by improved the traditional male urinary catheter.Methods Inspired by using ‘condom’‘fresh-keeping bags’and other pseudo catheter in clinic,the tradi-tional male urinary catheter was improved as‘pseudo catheter ’and the hydrocolloid dressing was used as bonding device.Results An im-proved male urinary catheter was successfully designed,which applied in 68 patients without urinary tract infection,catheterization failure nor foreskin edema.Conclusion The improved male urinary catheter has the advantages of less cost,simple operation,no pain,higher patient com-pliance and less complications,wihch is worthy to spread in clinic,especially in young patients and long-term bedridden elderly patients at home.

Objective To evaluate the efficacy of immediate and delayed postoperative removal of urethral catheters Methods Randomized controlled trials and quasi-experiment trails of immediate and delayed postoperative removal of urethral catheters were collected in the databases such as CENTRAL, PubMed, OVID, China National Knowledge Infrastructure (CNKI), China Biology Medicine disc (CBM), VIP, Wanfang Data, etc. Data were analyzed by RevMan 5.3 version.after heterogeneity analysis,pooled risk ratio (RR) with 95%confidence interval (95%CI) using both fixed and random effect models were estimated. Results Ten randomized controlled trials (RCTs) were included. The results of Meta-analysis indicated that immediate urinary catheter removal was associated with a lower incidence of urinary tract infections (RR=0.29, 95%CI 0.15 to 0.56, P=0.000 3), positive urinary culture (RR=0.31,95%CI 0.16 to 0.59, P=0.000 2) compared with the delayed removal group and seven studies recommended removal of the indwelling urinary catheter immediately after hysterectomy. While, there was a higher risk of re-catheterization (RR=4.56, 95%CI 2.93 to 7.08, P<0.01)in the immediate removal group. Seven studies recommended removal of the indwelling urinary catheter immediately after hysterectomy. Conclusions Removal of the indwelling urinary catheter immediately after hysterectomy was recommended.

Objective To summarize the meticulous nursing experience of indwelling urinary catheter between the children with hypospadias during perioperative. Methods 317 cases of indwelling urethral only in the silicone Foley tube, and without other urinary diversion with hypospadias were retrospectively analyzed, summarized the fine points of nursing, including preoperative children with comprehensive risk assessment, standardized preoperative skin preparation and postoperative protective constraint, effective properly fixed catheter keep the urine drainage unobstructed, close observation of urine drainage characteristics, maintain urine collection system tightness, maintain perineal skin clean and improved extubation nursing method, and do a good job after extubation nursing. Results 266 cases were cured with a cure rate of 83.9%(266/317);the incidence of urine leakage in 42 cases, the incidence rate was 13.2%(42/317); 7 cases of urethral stenosis, the incidence rate was 2.2%(7/317); 2 cases of urethral closing, the incidence was 0.6% (2/317), none of the patients had accidental extubation. Conclusions The meticulous nursing has a better effect in hypospadias patients during the perioperative period and it can reduce the incidence of postoperative complications;shorten the time of hospitalization and alleviate the suffering of children;improve the quality of nursing service, which is worthy of clinical application.

ABSTRACT

BACKGROUND: The reduction of orbital blowout fracture primarily aims to normalize the extra-ocular movement by returning the herniated orbital soft tissue into the original position, and to prevent enophthalmos by normalizing the orbital cavity volume. We introduce a balloon catheter-assisted orbital floor reduction technique. METHODS: A retrospective review was performed for all patients with orbital floor fracture who underwent the technique described in the main body of this text. Medical records were reviewed for demographic data, clinical presentation and course, degree of enophthalmos, intraorbital volume on computed tomography scan, and postoperative outcomes. The enophthalmos and intraorbital volume of the injured site were compared to the uninjured eye and orbit. RESULTS: The review identified 14 patients (11 male, 3 female). The mean preoperative difference in en-exopthalmos was 2.13 mm, while the mean orbital volume was 116%. The mean postoperative difference in en-exophthalmos had improved to 0.61 mm with a mean orbital volume of 101.85%. At the time of catheter removal at 10 days, three patients experienced diplopia (n=1), extra-ocular movement disorder (1), or enophthalmos (1). All of these had resolved by the 6-month follow-up visit. CONCLUSION: Balloon catheter-assisted reduction of the orbital floor fractures was associated with improvements in intraorbital volume and enopthalmos in the 14 patients. Notable complications included diplopia, enophthalmos, and limited extra-ocular movement, all of which were transient in the early postoperative period and had resolved by 6-month follow up.

ABSTRACT

Objective To evaluate the postoperative pain induced by various therapeutic catheters after abdominal surgery.Methods A prospective study was conducted in patients selected based on the inclusion criteria.The general condition of the patients was recorded,and nutritional risk screening was performed.The indwelling of therapeutic catheters after abdominal surgery were recorded,including urinary catheter,nasogastric tube,peritoneal drainage tube,common bile duct drainage tube,wound drainage tube,central venous catheter and peripherally inserted central catheter.The pain caused by each type of catheters was evaluated using visual analog scale at 24,48 and 72 hours after tube/catheter insertion.Results A total of 157 patients were selected,including 70 males and 87 females,aged (60.5 ± 12.5) years,with a body mass index of (23.8 ± 3.2) kg/m2,and a total nutritional risk rate of 42%.According to visual analog scale scores,the degrees of pain due to the therapeutic catheters,in descending order,were as follows:4.9 ± 1.7 for nasogastric tube,3.6 ± 0.9 for wound drainage tube,3.0 ±0.9 for urinary catheter,2.6 ±0.9 for central venous catheter,2.4 ± 1.0 for peritoneal drainage tube,1.9 ± 0.7 for common bile duct drainage tube,and 1.8 ± 0.8 for peripherally inserted central catheter.The catheter-induced pain accounted for (44.9 ± 14.1)% of the total pain during the hospital stay.Conclusions Nasogastric tube,wound drainage tube and urinary catheter can increase the pain of patients.It is therefore recommended to remove the indwelling tubes as early as possible if only the removal does not harm the outcome of the patient.
